Badly installed fibre patch panels
If a fibre cable is not fixed securely within the gland of the patch panel the fibre can be pulled or snagged and broken when other work is being carried out within the communications cabinet.

There are many reasons why fibre gets damaged along its route.
The usual reasons are due to cables being dug up during excavation works, pulled down from overhead by high sided vehicles or rodent damage.
Using optimum time domain reflectometer (OTDR) technology PSP can pinpoint a fibre breakage within 1.8 metres of the break.
When the breakage has been identified we can field joint the cable using portable fusion splice equipment ensuring minimal db loss. This means that when we re test the fibre it will still meet the industry standards and the fibre bandwidth will not be compromised.
